Social Work at Overgate Hospice
The Social Work Team at Overgate Hospice acknowledge that the diagnosis of a life-limiting illness will affect many areas your life, as well as the lives of those close to you.
We offer a range of services which we hope will address some of the difficulties encountered by patients and carers.
We aim to provide emotional, practical, financial and social support for In-patients, Day Hospice patients as well as families and carers.
Out Social Work Team is available to provide information and advice on a broad range of issues, including:
- Benefits/grants advice
- Housing
- Holiday travel arrangements
- Wills/Lasting Power of Attorney
- Advanced decisions
- Advanced funeral planning
- Life story/memory boxes
- Advocacy
There are many other problems which are not included in this list but with which we would be happy to help.
Discharge Planning
Overgate Hospice is a Specialist Unit and is therefore unable to provide long term nursing care. The Social Work Team is available to assess care needs in partnership with patients, families and carers, provide information and advice to enable patients, families and carers to make informed choices regarding the care options available.
