REFERRAL INFORMATION

Acute Inpatient Admission

In the first instance, these referrals should be made via the Community or Hospital Specialist Palliative care teams, who help triage cases, and offer advice and support in the patient’s management while waiting for a bed to become available. 
 

Out of Hours (OOH)

For patients known to our services already, a call from the Community Nursing Team may be sufficient in some cases. An out of hours GP review may be necessary for consideration of whether there are reversible/treatable problems that could continue to be managed at home, or which would be better treated in hospital, even for palliative care patients. For patients who are not known to the Hospice, a senior medical review (e.g. OOH GP) is usually needed for OOH referral for an inpatient bed.  Admissions will be at the discretion of the On-Call Consultant in Palliative Medicine.

To make an urgent out-of-hours referral for admission, please call Overgate directly on 01422 379151.  You may hear a recorded message – please press 1 to be transferred to the nursing team.

 

Nurse-Led Care

Broadly, this care is for patients who are approaching the last days of life, who do not have complex medical needs, and whose preferred place of death is Overgate Hospice.
 
From community settings – if the Community Specialist Palliative Care team is involved with the patient, please speak to them first, otherwise please ring Overgate directly on 01422 379151.
 
From CHFT hospital setting – if the Hospital Specialist Palliative Care team is involved with the patient, please speak to them first, otherwise, please talk to the Discharge Co-ordinator who may be able to facilitate transfer to the Hospice for appropriate patients.

Respite Care 

If the person is known to Day Hospice – please speak to the Day Hospice Team on 01422 379151.

If the person is known to the Community Specialist Palliative Care Team – please speak to the relevant CNS.

If the person is not known to either of these teams, please phone 01422 379151 and ask to speak to the nurses on the Inpatient Unit.
 


Day Hospice

Referrals can be made by health and social care professionals caring for a patient in the community setting by calling 01422 379151.

To refer a patient to this service please complete a referral form, tick the “Day Hospice” box and email it to Day Hospice on cah-tr.referrals.overgatehospice@nhs.net or telephone the Day Hospice Team on 01422 379151.

  

Breath of Fresh Air

The Calderdale Community Respiratory Team is the only team that can refer to this service, and they use structured referral criteria.

 

Time to Think

A referral from your GP or Healthcare Professional is required for you to access this service.

 

Carers Group

When a patient is admitted to one of our services, the team usually invites the carer to attend this group, but if you wish to enquire please call the Day Hospice on 01422 379151.
