If you are known to the Inpatient Unit, Day Hospice, or Community Palliative Care Nurses, a member of staff can refer you to the Physiotherapist and/or the Occupational Therapists who will arrange to attend and discuss your concerns.
Any patient known to Overgate with a life-limiting illness can access this service. We are not limited to patients with a cancer diagnosis; we are also there for people with respiratory conditions such as Chronic Obstructive Pulmonary Disease (COPD), neurological conditions such as Motor Neurone Disease (MND) and cardiology conditions such as heart failure, no matter your stage of illness.
You will be reviewed by the therapist for as long as your needs dictate. Your treatment plan will be regularly reviewed and TOGETHER we will make changes where we identify if appropriate/necessary to do so. If input is required at home, a referral will be made to the community therapy team.
